Living Wills
Is one type of an advance directive instrument. It allows a person to document their wishes regarding medical choices that should be made for them when they are no longer able to make their own choices.
Usually the living will is partnered with the power of attorney for health care or health care proxy. All three of these instruments are usually witnessed or notarized.
A power of attorney for health care names an individual, also known as a proxy, to make the health care decisions for the principle person when they are no longer able to. Your proxy should be a person whom you trust and completely understands your wishes regarding medical treatment. They must be willing to take the responsibility of carrying out your wishes. Appointing the proxy, also referred to as a health care surrogate, can raise questions you might never considered.
Each state has different governing laws, it is important to review them and comply with your state's law. Advance directives have many different names, you need to what what your state names the directive.
